Managing the Cost of IVF – Information from Our Sacramento Area Center
At Northern California Fertility Medical Center, Inc. (NCFMC), we know that the cost of IVF is significant and that many of our Sacramento and Northern California patients have well-founded concerns about paying for treatment that may or may not work. That is why we are pleased to offer a group of financial options for those patients who need In Vitro fertilization (IVF). Because of our consistently high IVF success rates, we are able to offer a special plan that will refund a portion of the medical fees if a pregnancy is not achieved, as well as "package pricing" for multiple attempts of IVF. These plan fees are subject to change without notification.
Please be advised that these plans are designed to offer patients some degree of indemnification against failure as opposed to a "guarantee" of success. In addition, treatment recommendations for plan participants are not different than for those who elect other financial options. Specifically, ovarian stimulation protocols are not more aggressive and recommendations for the number of embryos to be transferred are not influenced by which plan is chosen.
Payment for treatment with IVF (using non-donor or donor eggs) may also be "fee for service." The cost of IVF with non-donor eggs at Northern California Fertility Medical Center, near Sacramento includes the operating room and anesthesia fees, but not the costs of pre-cycle testing or medication. The cost of IVF with donor eggs also includes the operating room and anesthesia fees, but does not include additional donor fees, such as donor compensation, insurance, testing, and medication.
IVF Refund Plan (Plan One)
The IVF Refund Plan (Plan One) refunds 60 percent of the plan fee ($12,800) if a 12-week pregnancy is not achieved following a treatment cycle. A treatment cycle includes all of the in-cycle monitoring, the oocyte (egg) retrieval, sperm preparation and oocyte insemination, the embryo transfer, surplus embryo cryopreservation (freezing) and storage (for the first year), and two pregnancy tests, provided these services take place at Northern California Fertility Medical Center. If a pregnancy is not achieved following transfer of the fresh embryos, Plan One also includes transfer(s) of frozen/thawed embryos until a pregnancy is achieved or until all of the embryos have been transferred. Qualifying couples may use this plan for up to three different treatment cycles using a fresh embryo transfer.
Fees Not Covered by Plan One
- Pre-cycle lab work and testing
- The cost of IVF-related medications and/or the administration thereof
- Anesthesia and operating room fees (for the egg retrieval)
- Fees for intracytoplasmic sperm injection (ICSI) and epididymal or testicular sperm extraction when applicable
- Fees for blastocyst culture or assisted hatching when applicable
- Egg donor, surrogate, and agency fees when applicable
- Obstetric ultrasounds
- The cost of IVF services not provided by Northern California Fertility Medical Center
Groups Who May Use Plan One
- Group One: Women 39 and under using their own eggs
- Group Two: Women using donated eggs (the egg donor must be 34 or younger)
- Group Three: Women using gestational surrogacy

Criteria for Plan One
- When the patient's own eggs are used, the cycle of treatment must begin prior to the woman reaching the age of 40.
- For those in Group Two, the cycle of treatment must begin before the egg donor reaches the age of 35.
- The female partner must have a follicle stimulating hormone (FSH) blood level of less than 10 mIU/ml and an estradiol (E2) blood level of less than 70 pg/ml on the second or third day of a natural menstrual cycle. If a clomiphene citrate challenge test has been performed, the FSH level on day 10 must also be less than 10 mIU/ml.
- When donor eggs are used, the donor must have an FSH level less than 10 mIU/ml and an estradiol (E2) blood level of less than 70 pg/ml on the second or third day of a natural menstrual cycle.
- The female partner (or surrogate) must have a normal uterine cavity as assessed by hysteroscopy.
- The male partner must produce sperm in his ejaculate. Alternatively, if there are no sperm (azoospermia), he must have normal blood FSH (less than 12 mIU/ml) and testosterone levels, and be willing to undergo surgical testicular sperm extraction. Donor sperm may also be used.
- The couple must be willing to comply with all medical directives related to treatment, including cryopreservation of embryos (if available) and eventual transfer of all embryos derived from a single egg retrieval procedure.
- The couple must sign a contract.
- Couples may use Plan One up to three treatment cycles. Each treatment cycle must be approved by an NCFMC physician after evaluating performance in the prior cycle(s) and the financial contract must be renewed.

Prepaid Multiple IVF (Plan Two)
This plan is designed to provide IVF candidates with the opportunity to undergo two cycles of treatment for a reduced fee ($11,800)
Criteria for Plan Two
- In vitro fertilization must be medically indicated.
- The female partner must have a normal uterine cavity as assessed by hysteroscopy.
- All criteria outlined in the contract must be met.
- Couples who qualify for Plan Two pay a one-time, non-refundable fee for two fresh IVF attempts over a period of 12 months, the cryopreservation (freezing) of surplus embryos, and as many embryo transfer cycles necessary to transfer all previously frozen embryos.
- The obligation to provide continued services under this plan expires upon confirmation of a normal pregnancy that has proceeded to the twelfth gestational week or upon completing two fresh IVF and associated frozen/thawed embryo transfer cycles.
Fees Not Covered in Plan Two
- Pre-cycle lab work and testing
- The cost of IVF-related medications and/or the administration thereof
- Anesthesia and operating room fees (for the egg retrieval)
- Fees for intracytoplasmic sperm injection (ICSI) and epididymal or testicular sperm extraction when applicable
- Fees for blastocyst culture or assisted hatching when applicable
- Obstetric ultrasounds
- The cost of IVF services not provided by Northern California Fertility Medical Center

Shared Egg Donor Program
The shared egg donor program allows for a substantial reduction in the cost of IVF with egg donation without significantly influencing success rates.
In this program, couples pay a non-refundable fee for a single IVF/egg donation cycle in which two couples share the eggs obtained from a single donor (whom they both choose).
This fee includes:
- Donor compensation
- Donor precycle testing
- Anesthesia and operating room fees
The fee does not include:
- Fees for intracytoplasmic sperm injection (ICSI) and epididymal or testicular sperm extraction when applicable
- Embryo cryopreservation (freezing) when applicable
- Pre-cycle testing for the recipient couple
- Medications for the donor and the recipient couple
- Fees for blastocyst culture or assisted hatching when applicable
Criteria for the Shared Egg Donor Program
- IVF with egg donation must be medically indicated.
- The female partner (recipient) must have a normal uterine cavity as assessed by hysteroscopy.
- The egg donor must be under 35 years of age.
- All criteria outlined in the contract must be met.
Financial Responsibility and Insurance
The fee schedules which apply to the performance of single cycles of IVF, IVF/egg donation, and IVF/gestational surrogacy are available upon request. The cost of IVF-related medications (Lupron Depot®, Gonal-f®, Follistim®, Repronex®, Profasi®, and others) is not included in the fee schedule. The couple/individual will be required to purchase these independently.
Our Policy Regarding Reimbursement for Services Rendered
- We require payment in advance as delineated in the applicable fee schedule.
- In cases where advance payment has been made for a cycle of treatment involving an assisted reproductive procedure and the cycle is discontinued prematurely, the couple/individual will be refunded for services that have not been rendered.
- Please note that payments can be made by check, credit card, or in cash.

Our Policy Relating to the Acceptance of Insurance Assignments
- We do not bill the insurance company for assisted reproductive services in the absence of pre-authorization and, accordingly, the couple/individual is responsible for full payment in advance of such cases.
- The fee plans (refund or multi-cycle) cannot be billed to insurance.
It is helpful to carefully evaluate your particular health insurance policy in order to determine which procedure(s) are covered or excluded.
